This book provides the individual with the means to document and carry with them their medical details. Ideal for the corporate traveller or companies looking to augment their occupational health commitment. Although many physician records are computerised these days, they are not accessible by hospitals, pharmacies or other doctors. They are not portable and medical details including medications are often reliant solely on the individual's memory.

'Your Personal Health Record' is a booklet devised & designed by Dr Marcus Hawkins (BM BS) & Catherine Schmidt (RN), of Auckland, New Zealand. They have interests in medical documentation & communication between health professionals & patients & encouraging individuals to take a proactive role in their health.
